Responsibilities include, but are not limited to:
- Conducting thorough assessments of individuals in need of care, considering their physical, emotional, and social needs.
- Collaborating with service users, families, and other professionals to develop comprehensive care plans that promote independence and well-being.
- Advocating for service users' rights and choices in accordance with the Care Act.
- Providing emotional support and practical assistance to individuals and their families, ensuring their voices are heard and respected.
- Implementing safeguarding procedures to protect vulnerable individuals from abuse or neglect.
- Working closely with other agencies to investigate and address safeguarding concerns.
- Ensuring that care plans are implemented effectively, monitoring and adjusting as needed.
- Maintaining accurate and up-to-date records of assessments, care plans, and interventions.
Requirements:
- Degree in Social Work and registration with Social Work England.
- In-depth knowledge of the Care Act 2014 and relevant legislation.
- Previous experience working as a social worker in adult services.
- Strong assessment and analytical skills.
- Excellent communication and interpersonal skills.
- Enhanced DBS dated within the last 12 months. On the update service? You could be fast tracked through our recruitment process today.
