We are looking for a dedicated Health Coach to support individuals and small groups in making sustainable healthier lifestyle choices. Using an asset-based approach, the Health Coach will work in various environments, empowering people to make positive changes that enhance their overall health and well-being. This role involves carrying out assessments, creating personalized health plans, setting goals, and conducting physical health checks where required. A key aspect of the role is building strong relationships with local community assets and partners to ensure long-term positive outcomes.
Key Responsibilities:
- Use coaching techniques to support individuals and small groups in making sustainable health and lifestyle choices.
- Conduct assessments and develop individualized health plans and goals based on personal needs.
- Where required, undertake physical health checks to support overall well-being.
- Provide guidance on nutrition, physical activity, mental well-being, and healthy behaviors.
- Develop and maintain strong relationships with community organizations, healthcare providers, and support networks.
- Promote a person-centred approach, ensuring all coaching is tailored to the individual’s circumstances and aspirations.
- Encourage self-care, empowering individuals to take control of their health.
- Maintain accurate records and reports, ensuring compliance with relevant policies and procedures.
Key Skills & Experience:
- Experience in health coaching, wellness coaching, public health, or a similar role.
- Strong communication and motivational skills to support behavior change.
- Understanding of health promotion, lifestyle interventions, and community-based support.
- Ability to build positive relationships with individuals and external partners.
- Knowledge of local community assets and available support services.
- Experience in developing health plans and setting realistic goals for individuals.
Qualifications:
- Relevant qualification in Health Coaching, Public Health, Nutrition, Psychology, or a related field (or equivalent experience).
- Enhanced DBS check (Disclosure and Barring Service) required.
- Must be eligible to work in the U.K.
