Responsibilities include, but are not limited to:
- Conducting comprehensive assessments of individuals with learning disabilities to identify their strengths, needs, and preferences.
- Collaborating with individuals, families, and relevant stakeholders to develop person-centred support plans.
- Advocating for the rights and interests of individuals with learning disabilities within the community and various service settings.
- Providing emotional support and practical assistance to individuals and their families.
- Collaborating with multidisciplinary teams, including healthcare professionals, educators, and community organisations, to ensure coordinated and holistic support.
- Facilitating access to appropriate services, including healthcare, housing, education, and employment opportunities.
- Responding promptly to crisis situations and provide immediate support to individuals and families in distress.
- Developing crisis intervention plans and collaborate with relevant agencies to ensure ongoing safety and well-being.
- Maintaining accurate and up-to-date records of assessments, support plans, and interventions.
Requirements:
- Degree in Social Work or a related field.
- Registration with the Social Care Wales.
- Experience working with individuals with learning disabilities.
- Strong communication and interpersonal skills.
- Knowledge of relevant legislation and policies related to learning disabilities.
- Enhanced DBS dated within the last 12 months. On the update service? You could be fast tracked through our recruitment process today.
