Outreach Family Support Worker

Job description

Job purpose:

  • Taking whole case responsibility to provide high quality, outreach and home-based Early Help support to children and families using a 'whole family' approach (level 2 and 3).
  • Delivering targeted support to children across age ranges (0 -25) and their families in order to prevent the need for statutory services and to ensure families access Universal services.

Responsibilities include, but are not limited to:

  • Offering support on cases that become more complex and reach level 4.
  • Providing intensive and targeted support to vulnerable children and young people. This includes bespoke packages and programmes. This also includes supporting adolescents with complex emotional and mental health problems.
  • Supporting families/carers in developing practical parenting strategies which enhance positive parent/carer-child relations. This can include facilitated meetings between family members.
  • Attending regular supervision sessions with the Consultant Social Worker and provide evidence that work is progressing and that plans are improving outcomes for children and young people.
  • Meeting the requirements set out in the Early Help Hub Practice Standards.
  • Attending staff meetings, reviews, planning meetings and conferences and contributing towards plans and objectives for children and young people.
  • Contributing to evaluation and development of services and new ideas by sharing knowledge about theory, skills and practice with other Children's Services staff, professional groups and interested bodies.

Requirements:

  • NVQ Level 3 in childcare and education, NND, Health and Social Care qualification.
  • Experience of direct work with children, young people and their families.
  • Experience of effective collaborative and multi-agency working to support children and families.
  • Ability to communicate effectively, using good written and verbal skills with children, families, other professions and multi-disciplinary organisations in a wide range situation and at different levels.
  • Experience of delivering outreach and home based support to children and families to address areas of identified need.
  • Ability to maintain accurate, reflective records in accordance with Recording with Care guidelines.
  • To work flexibly to support the overall operation of the Service.
  • Enhanced DBS dated within the last 12 months. On the update service? You could be fast tracked through our recruitment process today.
