We are looking for a compassionate and dedicated Person-Centred Support Worker to join the team in Hounslow. The successful candidate will provide personalised support to individuals, promoting independence, dignity, and well-being. This role requires a caring individual with excellent communication skills and a commitment to enhancing the quality of life for those they support.
Responsibilities include, but are not limited to:
- Providing tailored support to individuals based on their unique needs and preferences.
- Assisting with daily living activities, including personal care, meal preparation, and household tasks.
- Supporting individuals in accessing community resources and participating in social activities.
- Developing and implementing person-centred support plans in collaboration with individuals and their families.
- Regularly reviewing and updating support plans to reflect changing needs and goals.
- Encouraging and empowering individuals to make informed choices and take control of their own lives.
- Assisting individuals in developing and maintaining social networks and friendships.
- Promoting independence by teaching life skills and encouraging self-advocacy.
Requirements:
- A relevant qualification in health and social care (e.g., NVQ Level 2 or 3) is preferred but not essential.
- Experience in providing person-centred support, preferably within a similar role.
- Experience working with individuals with learning disabilities, mental health issues, or physical disabilities is advantageous.
- Strong organisational skills and the ability to manage a varied workload.
- Ability to work both independently and as part of a team.
