Working as a Social Worker in the Early Intervention and Prevention Hub, you will provide strengths-based, outcome-focused, and enabling support to adults with complex needs requiring initial, time-specific interventions. Collaborating within a multidisciplinary team, you will assess needs and develop and implement care and support plans, including care and treatment plans where appropriate.
Responsibilities include, but are not limited to:
- Holding and managing a caseload, working collaboratively with individuals, carers, families, professionals, and community organisations.
- Employing a strengths-based and outcome-focused approach.
- Conducting high-quality assessments of needs in partnership with relevant professionals, guiding the provision of advice, support, or care plans.
- Developing strengths-based care and support plans for individuals with eligible needs, ensuring alignment with what matters to them.
- Working within the Early Intervention and Prevention Hub to manage high-risk and complex cases.
- Planning and commissioning responses to minimise risks to independence and meet identified needs.
- Monitoring and reviewing outcomes achieved, focusing on sustained independence and wellbeing.
- Participating in supervision and personal development reviews, identifying training needs to maintain professional registration.
Requirements:
- Degree in Social Work or equivalent qualification.
- Registration with Social Care Wales (or the equivalent governing body).
- Strong understanding of legislative and policy frameworks governing social work practice.
- Demonstrated ability to work collaboratively within multidisciplinary teams.
- Excellent communication, assessment, and planning skills.
- Commitment to safeguarding and promoting wellbeing.
