We are seeking a compassionate Support Worker to work alongside families, children, and young people in a person-centred way, helping them progress toward independent living. This role involves providing practical and flexible support, ensuring individuals can achieve their goals as outlined in their support and safety plans. The Support Worker will also contribute to support planning reviews and assessments through effective communication with the Support Coordinator or line manager.
Key Responsibilities:
- Work collaboratively with children, young people, and families to support their journey toward independence.
- Assist with practical tasks tailored to their needs and aspirations, as identified in their support and safety plan.
- Provide flexible and responsive support, adapting to evolving needs and challenges.
- Implement support and safety plans, ensuring they align with the goals set by the Support Coordinator.
- Communicate effectively with the Support Coordinator or line manager, contributing to support plan reviews and assessments.
- Empower individuals to develop essential life skills, access resources, and achieve their desired outcomes.
- Maintain accurate records and reports, ensuring compliance with safeguarding policies and procedures.
Key Skills & Experience:
- Experience supporting children, young people, and families in a similar role.
- Strong understanding of person-centred support and independent living principles.
- Ability to work flexibly and proactively, adapting to diverse needs.
- Excellent communication and interpersonal skills to engage effectively with individuals and professionals.
- Knowledge of safeguarding, social care policies, and support planning processes.
Qualifications:
- Relevant qualification in Social Care, Youth Work, Psychology, or a related field (or equivalent experience).
- Enhanced DBS check (Disclosure and Barring Service) required.
- Must be eligible to work in the U.K.
