Band 6 Physiotherapist

We are seeking a skilled and experienced Band 6 Physiotherapist to join a dedicated Adult Community Therapy Team. This role involves providing high-quality therapy interventions for a diverse range of adult patients with various conditions in the community, as well as offering therapy services for patients in Pathway 2 beds.

Responsibilities include, but are not limited to:

• Assessing, planning, and delivering physiotherapy interventions for patients with a wide range of conditions in community settings.
• Providing therapy services in Pathway 2 beds, promoting patient recovery and independence.
• Collaborating with a multidisciplinary team to deliver person-centred care.
• Monitoring, evaluating, and documenting patient progress, adjusting treatment plans as needed.
• Providing advice and education to patients, carers, and families to facilitate continued recovery and rehabilitation.

Requirements:

• HCPC registration as a Physiotherapist.
• Proven experience managing a wide range of conditions in a community setting.
• Strong assessment and clinical reasoning skills.
• Ability to work independently and collaboratively within a multidisciplinary team.
• Excellent communication and organisational skills.
• Enhanced DBS dated within the last 12 months. On the update service? You could be fast tracked through our recruitment process today.
