Competency Nurse

Job description

As a Community Complex Care Nurse, you will be responsible for delivering high-quality nursing care to clients with complex health needs in their own homes and within the community. This role involves providing care management, clinical interventions, and health assessments, ensuring each patient receives tailored support to enhance their quality of life. You will be required to travel across Essex, delivering care at various locations.

Responsibilities include, but are not limited to:

  • Providing nursing care and clinical assessments to patients with complex health conditions, including long-term illnesses, disabilities, and end-of-life care.
  • Administering medications, managing wound care, and performing clinical interventions as per care plans.
  • Monitoring patient progress and making necessary adjustments to care plans in collaboration with the multidisciplinary team.
  • Ensuring all care delivered is in line with current evidence-based practice and regulatory standards.
  • Working alongside healthcare professionals, such as doctors, social workers, and therapists, to create and implement effective care plans.
  • Supporting families and caregivers by providing education, advice, and guidance in managing patients’ conditions at home.
  • Maintaining accurate, up-to-date documentation of care delivered, including daily reports, assessments, and incident logs.
  • Adhering to all relevant regulatory and professional standards, including safeguarding, infection control, and health & safety regulations.

Requirements:

  • Registered Nurse (RN) with a valid NMC Pin.
  • Proven experience in providing nursing care within the community, particularly for patients with complex care needs.
  • Competent in administering medication and performing clinical interventions independently.
  • Experience with care planning, case management, and working within a multidisciplinary team.
  • Strong communication and interpersonal skills to work effectively with patients, families, and other healthcare professionals.
  • Full UK driving license and access to own vehicle.
  • Enhanced DBS dated within the last 12 months. On the update service? You could be fast tracked through our recruitment process today.
