GP Pharmacist

Job description

We are seeking an experienced and dynamic GP Pharmacist to join a dedicated healthcare team. The ideal candidate will bring expertise in medication management, patient consultations, and clinical services, supporting the practice in delivering high-quality, patient-centred care. This role involves collaboration with healthcare professionals, quality improvement initiatives, and a focus on medicines optimisation to enhance clinical outcomes.

Responsibilities include, but are not limited to:

  • Conducting comprehensive medication reviews for patients.
  • Managing repeat prescriptions, ensuring safe and effective medicine use.
  • Addressing acute prescription requests and managing medicines reconciliation during care transitions.
  • Undertaking structured clinical medication reviews and make evidence-based recommendations.
  • Reconciling medications post-hospital discharge or intermediate care and ensure continuity of supply for high-risk patients.
  • Developing and implementing repeat prescribing policies and oversee the reauthorization process.
  • Identify high-risk patient cohorts through practice computer searches, assessing risks related to medicines and patient conditions.
  • Monitoring prescribing against local and national guidelines and providing recommendations for shared or specialist care.
  • Collaborating with community and hospital pharmacies to improve patient outcomes and manage workloads effectively.

Requirements:

  • M.Pharm or BSc/B.Pharm degree.
  • Minimum of 2 years post-registration experience in healthcare settings.
  • Experience working within a multidisciplinary team.
  • Registration with the General Pharmaceutical Council (GPhC).
  • Experience in clinical audits and quality improvement initiatives.
  • Strong communication and collaboration skills for multi-agency working.
  • Enhanced DBS dated within the last 12 months. On the update service? You could be fast tracked through our recruitment process today.
