Physiotherapist

The TRI team offers a short-term, therapy-led, TEC-first reablement service for individuals within hospital discharge pathways and the community. This role involves managing a varied caseload, providing case management and specialist physiotherapy input in collaboration with domiciliary care providers and care home staff (residential reablement).

Responsibilities include, but are not limited to:

• Providing comprehensive case management and specialist physiotherapy services.
• Working alongside domiciliary care providers and care home staff to deliver residential reablement.
• Conducting specialist assessments, including falls risk assessments, and developing goal-oriented discharge plans.
• Supplying necessary equipment to support reablement and independence.
• Implementing specialist interventions to enhance patient outcomes.

Requirements:

• Degree in Physiotherapy.
• Registered with the Health and Care Professions Council (HCPC).
• Demonstrated experience in adult physiotherapy, preferably within reablement or community settings.
• Awareness of Technology Enabled Care (TEC) devices or a willingness to learn and incorporate these into practice.
• Strong assessment and clinical reasoning skills.
• Excellent communication and interpersonal skills.
• Ability to work independently and as part of a multidisciplinary team.
• Enhanced DBS dated within the last 12 months. On the update service? You could be fast tracked through our recruitment process today.
