As a Social Prescriber, you will be playing a crucial role in connecting individuals with non-medical resources and support in their local community. The successful candidate will be aiming to improve the overall well-being of individuals by addressing social determinants of health and fostering community engagement.
Responsibilities include, but are not limited to:
- Conducting thorough assessments to understand the social, emotional, and practical needs of individuals.
- Engaging in one-on-one consultations to develop a holistic understanding of clients' circumstances.
- Identifying and connecting clients to a wide range of local community resources, services, and activities that can address their specific needs.
- Collaborating with community organisations, social services, and healthcare professionals to build a network of support for clients.
- Developing personalised support plans in collaboration with clients, setting realistic goals and milestones.
- Providing ongoing guidance and encouragement to help clients achieve their objectives.
- Building and maintaining strong relationships with local community groups, charities, and organisations to stay informed about available resources.
- Organising and facilitating group activities, workshops, and events to promote social connections and community involvement.
Requirements:
- Relevant qualification relating to Social Prescribing.
- Previous experience in a similar role, preferably within a healthcare or community organisation.
- Strong interpersonal and communication skills.
- Empathy and the ability to build trust with diverse individuals and communities.
- Knowledge of local community resources and services.
