Band 6 Community Occupational Therapist

We are seeking a skilled and motivated Band 6 Community Occupational Therapist to join a Community General Rehabilitation team in Dartford, Kent. This role involves working closely with patients in a general rehabilitation ward and conducting home visits to support their recovery and enhance their independence.

Responsibilities include, but are not limited to:

• Conducting comprehensive assessments to identify patients' physical, emotional, and social needs.
• Developing and implementing tailored rehabilitation programs to achieve individual goals.
• Providing advice and support to patients, families, and caregivers.
• Performing home visits to evaluate living environments and recommend adaptations as needed.
• Collaborating with a multidisciplinary team to ensure high-quality, patient-centred care.
• Maintaining accurate and timely documentation of patient progress and interventions.

Requirements:

• HCPC registration and a degree in Occupational Therapy.
• Demonstrated experience in general rehabilitation or similar roles.
• Must hold a valid driver's license and have access to a car for home visits.
• Enhanced DBS dated within the last 12 months. On the update service? You could be fast tracked through our recruitment process today.
