Band 6 Community Occupational Therapist

Job description

We have exciting opportunities for Occupational Therapists to join a dynamic Community Neuro & Stroke Service team in Bath!

Responsibilities include, but are not limited to:

  • Conducting comprehensive assessments of patients with neurological and stroke-related conditions to determine their needs, strengths, and areas for improvement.
  • Developing and executing individualised care plans that align with patients' goals, focusing on enhancing their functional abilities and independence.
  • Conducting regular home visits to assess the living environment, provide therapy, and make recommendations for adaptations or equipment to facilitate patients' independence and safety at home.
  • Implementing a range of therapeutic interventions, including task-specific training, cognitive rehabilitation, and upper limb therapy, tailored to the specific needs of each patient.
  • Educating patients and their families on the nature of their conditions, the therapeutic process, and strategies to manage their daily activities and improve their quality of life.
  • Identifying potential risks to patient safety and well-being and taking appropriate measures to mitigate these risks, including referral to other services when necessary.
  • Acting as an advocate for patients, ensuring their voices are heard in care planning and decision-making processes.
  • Assisting in navigating healthcare services and accessing additional support if needed.

Requirements:

  • Degree in Occupational Therapy.
  • HCPC Registration.
  • Experience working in a community setting, particularly in neuro and stroke rehabilitation, is preferred.
  • Must be able to drive and have access to a car for home visits.
  • Excellent communication ability.
  • Enhanced DBS dated within the last 12 months. On the update service? You could be fast tracked through our recruitment process today.
  • Willingness to complete an enhanced DBS if you do not currently hold one.

Pay:

  • Day Weekdays: £30.54 per hour
  • Day Saturdays: £33.15 per hour
  • Day Sundays: £33.15 per hour
  • Day Bank Holidays: £35.76 per hour
