Contact and Assessment Team Manager

Job description

Responsibilities include, but are not limited to:

  • To ensure that all services are delivered in a preventative and strengths-based way with a consistent approach to eligibility across the service area.
  • To ensure that the social work team staff have an excellent understanding of the wide range of help available to support and promote independence and wellbeing and the legal framework in place to address more complex matters.
  • To be responsible for the First Point of Contact Social Work and Duty functions, working in partnership with Independent Living Contact Officers and Occupational Therapists and other related partners to deliver an integrated triage service and ensuring that requests for social work support are dealt with promptly and effectively.
  • To be responsible for the allocation of work ensuring that citizens are supported by appropriately experienced and competent practitioners. To monitor and support effective systems to allocate based on professional decisions about risk and severity of issues.
  • To be responsible for the escalation of matters to the Service Manager where there are legal and other complexities.
  • To work in partnership with internal and external partners to ensure the best possible outcomes for citizens and to model partnership working to the staff group promoting an inclusive whole system approach.
  • To monitor and inform the Service Manager the management of complex cases and safeguarding issues and to maintain a clear oversight of legal applications, ensuring appropriate instructions are made to Legal Services and counsel as required.

Requirements:

  • Social Work qualification.
  • Registration with Social Care Wales.
  • A good understanding of the Social Services and Wellbeing (Wales) 2014, Mental Capacity Act 2005 and All Wales Safeguarding Procedures and other relevant legislation/ Best Practice Guidelines.
  • A good understanding of preventative and strengths based social work practice.
  • Experience of supervising social work staff, and or providing mentoring to colleagues.
  • Minimum of 3 years of post-qualifying experience within a social service.
